Insurance and Billing
We highly recommend that you verify your insurance benefits before your first appointment. You may use our benefits verification form, and bring it with you for your first visit. Patients who have insurance without verified coverage should expect to pay at the time of service. We send bills electronically to your insurance as a courtesy to our patients. However, verifying benefits, obtaining referrals, prior authorization, and resolving any payment issues is the patient's responsibility. As a small private practice we unfortunately do not have the resources to communicate with insurance companies outside of submitting claims.
